More Than a Hormone: Why Chasing Testosterone Numbers Won't Cure Your Erectile Dysfunction
When a man begins struggling with erectile dysfunction, one of the first explanations that comes to mind — and often the first thing a quick internet search suggests — is low testosterone. The logic feels intuitive: testosterone is the male sex hormone, erections are a male sexual function, therefore low testosterone must be the culprit. It is a clean narrative. It is also, for the majority of men with ED, an incomplete one.
Clinical researchers have spent decades examining the relationship between testosterone and erectile function, and the findings consistently challenge the popular assumption. Understanding what the science actually says can spare men considerable time, money, and frustration — and point them toward interventions that genuinely work.

A man who attributes his ED solely to testosterone and pursues only hormonal treatment may be overlooking a significant cardiovascular problem that requires its own attention. This is not merely a treatment strategy concern; it is a health safety concern.
What a Thorough Diagnostic Workup Should Include
Given this complexity, a responsible evaluation of erectile dysfunction should extend well beyond a single testosterone blood test. Physicians who specialize in men's sexual health typically assess several interconnected factors:
Hormonal profile: Total and free testosterone are relevant, but so are other hormones. Elevated prolactin can suppress sexual function. Thyroid dysfunction — both hypo- and hyperthyroidism — can contribute to ED. Estradiol levels matter as well, since excess estrogen can counteract testosterone's effects.
Cardiovascular and metabolic markers: Fasting blood glucose, HbA1c, lipid panels, and blood pressure readings provide a picture of vascular health. These numbers often tell a more actionable story than hormone levels alone.

Neurological evaluation: Conditions such as multiple sclerosis, Parkinson's disease, or spinal injuries can impair the nerve signals required for erection. A thorough history helps identify neurogenic contributors.
Psychological screening: Performance anxiety, depression, and relationship stress remain significant drivers of ED, particularly in younger men. These factors do not appear on a blood panel and require direct clinical conversation to uncover.
When Testosterone Treatment Is Genuinely Appropriate
None of this is to suggest that testosterone therapy is without value. For men with confirmed hypogonadism — typically defined as total testosterone below 300 ng/dL accompanied by symptoms — TRT can meaningfully improve quality of life, energy, mood, body composition, and sexual desire. In some men, particularly those whose ED has a significant libido-related component, hormone optimization is a legitimate and beneficial part of a broader treatment plan.
The critical word is part. Testosterone therapy is most effective when it is combined with other appropriate interventions: PDE5 inhibitors such as sildenafil or tadalafil for vascular-mediated ED, lifestyle modifications to address metabolic risk factors, and psychological support where relevant. Treating the hormone in isolation while ignoring the vascular and psychological dimensions is unlikely to restore full erectile function.
Building a Treatment Plan That Addresses the Whole Picture
For men who have been frustrated by testosterone therapy that did not deliver the erectile improvements they anticipated, the path forward involves a more comprehensive reassessment. This typically means consulting a urologist or men's health specialist rather than relying solely on a primary care provider who may have limited bandwidth to conduct a full evaluation.
It also means being candid about the full range of symptoms. Distinguishing between reduced sexual desire and difficulty with erection itself is clinically important — these are different problems with different solutions, even though they often coexist.
Finally, it means recognizing that erectile dysfunction is rarely a single-cause condition in men over 40.
